Job Description

We are seeking a highly skilled Discharge Case Manager to join our team. In this role, you will be responsible for coordinating pre-release or post-release case management and assessing the discharge planning needs of all inmates.

You will work under the direction of the Health Services Administrator or Program Manager to implement re-entry care plans and provide ongoing support to individuals served in accessing community behavioral health resources, substance abuse treatment, housing, employment, education assistance, and other relevant needs.

Your goal will be to ensure successful re-entry into society by coordinating community services.

Required Skills and Qualifications

To be successful in this role, you must have a bachelor's degree in social work. You should also possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.

You will need to demonstrate strong problem-solving skills, with the ability to analyze complex situations and develop effective solutions.
