The ideal candidate will possess strong digital literacy skills, demonstrate proficiency with telehealth platforms, and maintain a high standard of patient care and communication in a virtual setting.
Key Responsibilities:
- Foster trust-based relationships and proactive healthcare management with patients.
- Collaborate closely with other team members to develop, update, and prioritize patient care plans.
- Ensure team-driven adherence to these plans, focusing on holistic health outcomes, patient satisfaction, and effective resource utilization.
Requirements:
- Education: Successful completion of an approved Nurse Practitioner program (BSN required, MSN preferred).
- Licenses/Certifications: Current nursing license issued by the State of Maine or eligible for licensure, current BLS certification.
- Experience: Minimum three years of professional nursing experience (two years of Nurse Practitioner experience preferred).
