We are seeking a highly skilled and experienced RN Clinical Manager to join our team at CenterWell Home Health. As a Clinical Manager, you will be responsible for overseeing the delivery of high-quality patient care services in a home health setting.
Key Responsibilities:
- Develop and implement clinical operations strategies to ensure efficient and effective patient care delivery.
- Lead and manage a team of clinical staff, including nurses, therapists, and home health aides, to provide comprehensive patient care services.
- Conduct regular assessments and evaluations of patient care services to ensure compliance with regulatory requirements and Company policies.
- Collaborate with interdisciplinary teams to develop and implement patient care plans that meet individual patient needs.
- Monitor and analyze patient outcomes to identify areas for improvement and implement changes to enhance patient care services.
- Develop and maintain relationships with referral sources, including physicians, hospitals, and other healthcare providers.
- Participate in performance improvement activities to enhance patient care services and improve operational efficiency.
Requirements:
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
