Job Summary

We are seeking a skilled Certified Nursing Assistant to join our team at The Pennant Group, Inc. as a Hospice Care Assistant. In this role, you will provide high-quality patient care and support to individuals in need of hospice services.

Key Responsibilities
  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Provide emotional support and companionship to patients and their families.
  • Assist with medication management and administration.
  • Monitor patients' vital signs and report any changes to the nursing team.
  • Assist with transfers and ambulation as needed.
  • Provide education and support to patients and their families on disease management and end-of-life care.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
Requirements
  • Certified Nursing Assistant (CNA) certification in good standing.
  • Minimum one year of experience in hospice care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Valid driver's license and reliable transportation.
