Social Services Department Head

The Aspire Nursing and Rehab of Perry seeks an experienced Social Services Director to lead our social services department. This is a challenging role that requires strong leadership skills, excellent communication abilities, and a deep understanding of healthcare regulations.
Estimated Salary: $80,000 - $110,000 per year.
About the Role- This position is responsible for planning, developing, and directing the overall operation of the social services department.
- You will assist in the admission process, admission contracts, PAE, and PASSAR.
- You will assist in the development, administration, and coordination of department policies and procedures.
- You will keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator.
As the Social Services Director at Aspire Nursing and Rehab of Perry, you will be responsible for:
- Developing and implementing policies and procedures for the identification of medically related social and emotional needs of residents.
- Making arrangements for obtaining needed adaptive equipment, clothing, and personal items as necessary and appropriate.
- Maintaining contact with family members to report on changes in health, current goals, resident's rights, discharge planning, and to encourage participation in care planning.
- Making referrals and obtaining services from outside entities, such as talking books, absentee ballots, and community wheelchair transportation.
- Assisting residents with financial and legal matters, such as applying for pensions, referrals to attorneys, and referrals to funeral homes for preplanning arrangements, as necessary.
- Completing discharge plans in accordance with discharge plan policy.
- Developing relationships between residents and staff and teaching staff to understand and support residents' individual needs.
- Promoting actions by staff that maintain or enhance each resident's dignity in full recognition of each resident's individuality.
