Medical Director for Chronic Disease Management

The OpportunityWe're seeking a highly skilled and experienced Medical Director to lead our Impact program, a chronic disease management initiative focused on cardiometabolic conditions. As a key member of our clinical leadership team, you'll be responsible for designing and implementing clinical programs, supervising a team of clinicians, and serving as an expert clinical resource for primary care providers.
Key Responsibilities:- Evaluate and treat patients during in-person, tele-health visits, and/or virtual group settings, addressing primary care needs while managing chronic conditions.
- Develop educational content and digital learning curricula for cardiometabolic conditions to support the Impact program.
- Partner with our Quality and Population Health team to improve performance on quality measures and design targeted outreach to those with care needs.
- Deploy and assess the use of remote patient monitoring devices.
- Monitor member/patient and provider feedback and performance on program metrics, with opportunities for scholarly publications.
- Board Certified in Family Medicine or Internal Medicine.
- Board certification in Lifestyle Medicine or Obesity Medicine required.
- At least 2 years of experience in an outpatient primary care setting, or 1 year in an outpatient primary care setting coupled with 1 year of primary care fellowship.
- Direct patient care experience in chronic disease management required.
- State licensed in California, New York, or Washington D.C.; compact license preferred.
- Willingness to work 40 hours per week, including 80% in-office/remote patient care and 20% administrative time.
- Able to work a minimum of 1 day in-office (in our CA, NY, or DC office).
- Program management experience leading cross-functional teams.
- Demonstrated experience with the development, deployment, and ongoing evaluation of clinical curricula in Lifestyle Medicine, Obesity, or other Cardiometabolic conditions.
- Board certification in Advanced Diabetes Management (BC-ADM) or Certified Diabetes Care and Education Specialist (CDCES) obtained prior to or during employment with One Medical.
- A competitive salary range of $138,000 to $250,000 per year, based on a full-time schedule.
