- Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare teams.
- Provide skilled nursing visits, case management, and follow-up care to ensure optimal patient outcomes.
- Coordinate with multidisciplinary teams, including physical therapists, occupational therapists, and home health aides, to provide seamless care.
- Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
- Monitor patient progress, adjust care plans as needed, and document all interactions and interventions.
To succeed in this role, you will need:
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or acute care settings
- Strong med-surg, ICU, ER, and acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Excellent organizational and communication skills
- Valid driver's license, auto insurance, and reliable transportation
