  • Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing visits, case management, and follow-up care to ensure optimal patient outcomes.
  • Coordinate with multidisciplinary teams, including physical therapists, occupational therapists, and home health aides, to provide seamless care.
  • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
  • Monitor patient progress, adjust care plans as needed, and document all interactions and interventions.
Requirements and Qualifications

To succeed in this role, you will need:

  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or acute care settings
  • Strong med-surg, ICU, ER, and acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Excellent organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
