Job Summary
We are seeking a skilled Registered Nurse to join our team as a Clinical Nurse Specialist. In this role, you will be responsible for delivering high-quality patient care, collaborating with interdisciplinary teams, and promoting health education and support.
Duties and Responsibilities
- Develop and Implement Individualized Care Plans: Assess patients' physical, emotional, and social needs, develop tailored care plans, and collaborate with healthcare teams to implement and evaluate these plans.
- Conduct Comprehensive Health Assessments: Perform thorough assessments of patients' health status, identify potential risks and complications, and recommend interventions to prevent adverse outcomes.
- Provide Education and Support: Educate patients and families on health promotion, disease prevention, and management of chronic conditions, empowering them to take an active role in their care.
- Collaborate with Interdisciplinary Teams: Work closely with physicians, therapists, and other healthcare professionals to coordinate patient care, ensure seamless transitions between care settings, and promote optimal patient outcomes.
Required Qualifications
- Education: Bachelor of Science Degree in Nursing (BSN) or Master's Degree in Nursing (MSN)
- Experience: 1 year of experience in the related nursing specialty, preferably in a hospital setting
- Licenses and Certifications: RN License in the state of employment or compact, BLS required, ACLS within 6 months of hire/transfer date
