We are seeking a dedicated and compassionate Hospice Registered Nurse to join our team at Interim HealthCare. As a Hospice Nurse, you will play a vital role in providing end-of-life care to our patients and their families.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their hospice needs, including a history of current and previous illnesses, as well as a physical assessment.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Provide ongoing assessments of patients' physical and psychosocial status, and make adjustments to their care plans as needed.
- Graduate of an accredited Registered Nurse (RN) program.
- Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
- One (1) year of hospice care experience preferred.
