The estimated annual salary for this position ranges from $72,000 to $76,000, with attractive pay and benefits.

Job Description

As a care transitions coordinator, you will be responsible for educating healthcare professionals, patients, and families about home care services. You will develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices to generate leads. Your duties will include coordinating patient admissions, conducting on-site visits, and maintaining accurate records of patient interactions and referrals.

Required Skills and Qualifications

• At least one year of experience in a healthcare-related business development/outreach role
• Current RN or LPN license, specific to the state(s) assigned to work
• Reliable transportation, current driver's license, and liability insurance
