Overview:

Amedisys, a leading home health and hospice company, is seeking a Care Transitions Coordinator to join their team. This role requires an RN, LPN, or other clinical license, along with previous experience in healthcare business development or outreach. A background in sales, particularly in pharmaceuticals, medical devices, or hospital settings, is highly valued.

Responsibilities:

  • Educate healthcare professionals, patients, and families about home care services.
  • Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices, to generate leads.
  • Coordinate patient admissions with the team, including obtaining necessary documentation for the referral-to-admission process.
  • Conduct on-site visits to assess patient needs and identify barriers to care.
  • Keep accurate records of patient interactions and referrals.
  • Achieve admissions, product, and payor-mix goals.
  • Meet short and long-term target account goals.
  • Document sales calls daily in CRM, including pre and post-call notes.
  • Maintain a complete, up-to-date record of targeted referral sources in the territory in CRM.
  • Attend community events to promote Amedisys.
  • Respond to customer complaints in a timely manner.

Qualifications:

  • At least one year of experience in a healthcare-related business development or outreach role.
  • Current RN or LPN license, specific to the state(s) assigned to work.
  • Reliable transportation, current driver's license, and liability insurance.
