Key Responsibilities- Assess and Coordinate Patient Care: Conduct thorough assessments of patients to identify discharge needs and develop individualized plans to ensure timely and efficient care transitions.
- Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to remove barriers to care and ensure effective communication.
- Facilitate Care Transitions: Coordinate referrals for post-acute care, home health services, and other necessary resources to support patient recovery.
- Document and Communicate: Accurately document patient information and communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Lead and Participate in Quality Initiatives: Contribute to quality improvement efforts, participate in multidisciplinary rounds, and collaborate with other departments to promote best practices and patient safety.
- Education: Bachelor's degree in Nursing (BSN) or related field.
- Experience: Minimum 3 years of nursing experience, with a focus on case management or discharge planning.
- Certifications: Certification in Case Management (CCM) or related field.
- Skills: Strong clinical assessment skills, effective communication and interpersonal skills, and ability to work independently and collaboratively as part of a team.
