Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Registered Nurse Case Manager. As a member of our team, you will have the opportunity to provide exceptional patient care while enjoying the flexibility and autonomy that comes with working in home health care.
Key Responsibilities:- Manage a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Conduct home visits to assess patient needs and provide education and support to patients and their families.
- Document patient information and progress in a timely and accurate manner.
- Valid Nursing License in the State of New Hampshire.
- Nursing experience required.
- Current CPR Certification required.
