Pharmacist Per Diem Opportunity

The Department of Pharmaceutical Services at the University of California, San Francisco (UCSF) is seeking a highly skilled Pharmacist to join our team as a Per Diem Outpatient Pharmacist. This role will provide pharmaceutical care to patients at all outpatient pharmacy locations within the UCSF Health System.
Key Responsibilities- Provide pharmaceutical care to patients, including obtaining medication histories, reviewing and assessing medication profiles, and providing drug information to patients and healthcare providers.
- Collaborate with clinicians on drug selection and therapeutic plans, ensuring access to medications, and educating patients on proper medication use.
- Participate in the oversight of pharmacy technicians, pharmacy student interns, and pharmacy residents in their daily responsibilities.
- Assist in the oversight of clinical activities, delivery of services, and the Patient Management Program.
- Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy.
- Current license in good standing to practice pharmacy in the State of California.
- Basic Life Support (BLS) certification from the American Heart Association.
- Experience with navigating billing options for pharmacist care and completion of an ASHP-accredited PGY1 Pharmacy Residency or at least 3 years of equivalent experience in outpatient/specialty pharmacy services.
