Nurse Coordinator

We are seeking a highly skilled and experienced Lead Nurse to join our team at Long Island Select Healthcare Inc. (LISH). As a Federally Qualified Health Center (FQHC), we provide comprehensive healthcare services to our community, and we are committed to delivering high-quality patient care.
The successful candidate will be responsible for coordinating nursing care, including direct patient care, patient/family education, and transitions of care. They will support professional nursing practices and work collaboratively with our healthcare team to ensure the best possible outcomes for our patients.
The ideal candidate will have a strong background in nursing, with at least 3 years of direct patient care experience. They will be a positive and energetic individual who is detail-oriented and able to work effectively in a fast-paced environment.
Key responsibilities of the role include:
- Managing coverage for providers for nursing call outs
- Receiving and managing telephone calls
- Running and participating in daily huddles and managing review schedules
- Managing and maintaining waiting room, office, and exam rooms, as needed
- Monthly inspections of all emergency equipment and supplies, including expiration dates
- Performing duties as assigned by the Chief Medical Officer to support and ensure general medical operation of LISH
The successful candidate will also be responsible for providing relevant information about the PCMH program to patients, including brochures that describe the value and goals of the program. They will work to maintain a safe work environment and adhere to agency policies, procedures, rules, and regulations of confidentiality.
