Position Overview: The Team Leader for Care Coordination plays a crucial role in guiding, mentoring, and developing team members. This position involves participating in the recruitment process, training, coaching, and setting a positive example for staff. The Team Leader will also engage in daily problem-solving, manage staffing needs, and assist in addressing escalated client concerns.

Key Responsibilities:

  • Resolve staffing challenges related to absences and ensure optimal workflow.
  • Assign tasks effectively to maximize efficiency and support partner agencies.
  • Conduct regular supervision sessions with team members and maintain documentation.
  • Assist in the recruitment and selection of Care Coordinators.
  • Collaborate with partner organizations to meet the needs of shared clients.
  • Support the training and onboarding of new team members.
  • Provide backup support to management as required.
  • Maintain effective communication to identify and address potential challenges.
  • Deliver Care Coordination services across various locations as needed.
  • Foster a positive environment focused on customer service and client-centered care.

Qualifications:

Required:

  • High School Diploma or Equivalent.
  • Two years of experience in a behavioral health or physical health setting.
  • One year of supervisory or leadership experience.

Preferred:

  • Post-secondary education or specialized certification.
  • Experience in customer service.

Skills and Abilities:

  • Strong customer service skills and proficiency in active listening.
  • Excellent written and verbal communication abilities.
  • Familiarity with basic computer applications, including word processing and email.
  • Reliable transportation and a valid driver's license.

Work Environment: The role is primarily office-based, involving computer use for extended periods. Physical demands may include lifting and occasional travel.

Compensation: The hourly pay range for this position is competitive and commensurate with experience.
