Position Overview: The Team Leader for Care Coordination plays a crucial role in guiding, mentoring, and developing team members. This position involves participating in the recruitment process, training, coaching, and setting a positive example for staff. The Team Leader will also engage in daily problem-solving, manage staffing needs, and assist in addressing escalated client concerns.
Key Responsibilities:
- Resolve staffing challenges related to absences and ensure optimal workflow.
- Assign tasks effectively to maximize efficiency and support partner agencies.
- Conduct regular supervision sessions with team members and maintain documentation.
- Assist in the recruitment and selection of Care Coordinators.
- Collaborate with partner organizations to meet the needs of shared clients.
- Support the training and onboarding of new team members.
- Provide backup support to management as required.
- Maintain effective communication to identify and address potential challenges.
- Deliver Care Coordination services across various locations as needed.
- Foster a positive environment focused on customer service and client-centered care.
Qualifications:
Required:
- High School Diploma or Equivalent.
- Two years of experience in a behavioral health or physical health setting.
- One year of supervisory or leadership experience.
Preferred:
- Post-secondary education or specialized certification.
- Experience in customer service.
Skills and Abilities:
- Strong customer service skills and proficiency in active listening.
- Excellent written and verbal communication abilities.
- Familiarity with basic computer applications, including word processing and email.
- Reliable transportation and a valid driver's license.
Work Environment: The role is primarily office-based, involving computer use for extended periods. Physical demands may include lifting and occasional travel.
Compensation: The hourly pay range for this position is competitive and commensurate with experience.
