We are seeking a skilled Registered Nurse to join our team as a Care Coordinator. In this role, you will work closely with our primary care team to coordinate care services for our members. Your expertise and compassion will make a significant difference in the lives of our patients and their families.
Key Responsibilities- Establish and maintain effective communication with members, caregivers, and healthcare providers to ensure seamless care coordination.
- Review member status and adjust care plans as necessary to achieve optimal health outcomes.
- Perform administrative tasks, such as data collection and entry, to support care coordination efforts.
- Participate in program effectiveness analysis and planning to improve care coordination processes.
- Communicate with members, families, and caregivers to provide education and support.
- Current unrestricted Registered Nurse license in Oregon.
- 2+ years of RN experience, preferably in case management or post-acute care.
- Experience with electronic medical records and Microsoft Office.
- Valid driver's license and ability to travel locally 75% of the time.
- BSN degree.
- 1+ years of case management experience.
- Home Health experience.
- Experience working in post-acute care, such as long-term care or hospice.
