RN Care Manager - Full Time - Eddy SeniorCare

Job Description:

As a RN Care Manager with Eddy SeniorCare, you will play a vital role in managing a caseload of patients and helping them remain as independent as possible for as long as possible in the community. You will work closely with a comprehensive interdisciplinary team to provide high-quality care and support to our seniors.

Responsibilities:

- Manage a caseload of patients and provide individualized care and support
- Collaborate with the interdisciplinary team to develop and implement care plans
- Conduct regular assessments and evaluations to ensure the best possible outcomes for our patients
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

Requirements:

- NYS RN licensure
- Two or more years of experience in a healthcare setting
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
