Position Overview
We are seeking a qualified Statewide Travel Nurse Practitioner to join our dedicated team. This role will report directly to the AVP of Advanced Illness Management and will be responsible for delivering advanced illness management and palliative care services to our patients.
The selected Nurse Practitioner will be expected to support various locations throughout the state, ensuring high-quality care is accessible to all.
Key Responsibilities
- Develop personalized treatment plans and deliver comprehensive care in patients' homes, including diagnosis, treatment, consultation, and follow-up under the guidance of the Palliative Medical Director or Primary Care physician.
- Engage in extensive statewide travel, up to 80-90% of the time, to provide essential patient care across multiple branches.
- Occasional regional travel may be required, but will be limited to less than 10%.
- Flexible scheduling options are available to accommodate work-life balance.
Qualifications
- Master of Science in Nursing from an accredited institution, or equivalent qualifications as permitted by state law.
- A minimum of 2 years of experience as a Nurse Practitioner.
- Current registered nurse license and Nurse Practitioner licensure as mandated by state regulations.
- Active and unrestricted Nurse Practitioner certification/license.
- Multi-state licensure is preferred, or willingness to obtain additional state licenses (company-sponsored) for those interested in regional travel opportunities.
- Experience with Part B documentation and coding is essential.
- A collaborative agreement with a supervising physician is required, or the candidate must agree to establish such an agreement with the Palliative Medical Director as per state law.
- Part B Provider certification and Federal DEA license are necessary.
- State DEA license or specific state prescribing authority (if applicable).
- Certification in Hospice and Palliative Care (ACHPN) is preferred.
- Specialization in Family or Adult Medicine, or Acute Care is preferred.
- Willingness to travel extensively by car (up to 80-90%).
