**About the Role**
The Care Continuum Manager will work closely with hospital leadership and multiple departments to improve patient outcomes attributed to the Accountable Care Organization (ACO). This individual will monitor the ACO Care Continuum for patients discharged from ACO hospitals, managing local partners in the post-acute network. The Care Continuum Manager will collaborate with hospital leadership and case managers to review trends in resource utilization and serve as a subject-matter expert on ACO-related matters.
**Key Responsibilities**
* Collaborate with hospital leadership to develop and implement strategies to improve patient outcomes
* Work with multiple departments to monitor and manage the ACO Care Continuum
* Manage local partners in the post-acute network
* Review trends in resource utilization and provide insights to hospital leadership
**Requirements**
* Strong understanding of ACO principles and operations
* Excellent communication and collaboration skills
* Ability to analyze data and provide insights
