CAN Community Health is seeking a highly motivated and compassionate Patient Access Specialist 1 to join our team. As a Patient Access Specialist 1, you will play a critical role in facilitating access to care for individuals impacted by HIV and other infectious diseases.

Key Responsibilities:
  • Conduct thorough assessments of patients' social determinants of health and identify barriers to care.
  • Link patients to 340B, RW, and ACA services and eligibility options.
  • Provide ongoing patient support and advocacy to ensure continuity of care.
  • Collaborate with healthcare teams to coordinate patient services and ensure seamless transitions.
  • Stay up-to-date on HIV medical terminology, procedures, medications, and treatment practices.
Requirements:
  • College degree and/or three (3) years of relevant work experience.
  • Experience working with priority populations.
  • Strong interpersonal skills and ability to work effectively with diverse populations.
  • Knowledge of community health services and agencies.
  • Ability to accurately coordinate multiple tasks and work autonomously.
Preferred Qualifications:
  • Three (3) years plus in HIV programmatic experience (RW, ACA).
  • General Nursing, Medical assistant, Pharmaceutical Industry, Insurance, Public health, and/or Case Management experience in relevant industry.
  • Bilingual in Spanish or Creole.
