Job Responsibilities
  • Collaborate with healthcare team to coordinate services for members
  • Conduct psychosocial assessments to evaluate patient goals and needs
  • Develop and monitor plans of care to promote optimal functioning and quality of life
  • Identify and facilitate access to community resources and services
  • Ensure consistent and reliable documentation of case management activities
  • Analyze patient and program outcomes to identify improvements
  • Provide supportive counseling and education to members and healthcare team
  • Promote self-awareness and knowledge of current case management standards
Requirements
  • Minimum one year of clinical social work experience in a healthcare setting
  • Masters degree in social work required
  • Licensed Independent Social Worker (District of Columbia) within 6 months of hire
  • Licensed Master Social Worker (Maryland) within 6 months of hire
  • Licensed Master's Social Worker (Virginia) within 6 months of hire
Preferred Qualifications
  • Experience with computer software programs in a Windows environment
  • Knowledge of community systems and resources in the defined service area
  • Knowledge of regulatory issues for the Mid-Atlantic area
