Job Summary

We are seeking a skilled Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our team at Community Memorial Healthcare. As a member of our healthcare team, you will provide direct nursing care to residents in our Continuing Care Center under the supervision of a Registered Nurse.

Responsibilities
  • Provide direct nursing care to residents, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with the healthcare team to develop and implement individualized care plans for residents.
  • Conduct regular assessments of residents' physical and emotional needs, and make recommendations for care and treatment.
  • Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improving patient outcomes.
  • Maintain accurate and up-to-date records of resident care, including progress notes, medication lists, and treatment plans.
Requirements
  • Current California LVN or RN license
  • Current BLS certification
  • One or more years of experience in a long-term care facility
  • Preferred: IV skills and certification (LVN II)
