We are seeking a compassionate and empathetic Registered Nurse to join our team as a Hospice Care Coordinator - Palliative Support. This role is responsible for providing direct patient care services and supporting patients and their families in a collaborative environment.

About the Role

  • Provide high-quality, individualized care to hospice patients and their families.
  • Collaborate with an interdisciplinary team of healthcare professionals to enhance the quality of care.
  • Case manage all assigned patient clinical records.

Requirements

  • Current RN licensure in the state of employment.
  • A minimum of 1 year experience in oncology, long-term care, or hospice/home health.
  • Excellent communication and interpersonal skills.
