We are seeking a compassionate and skilled Hospice Care Provider to join our team at Interim HealthCare of South Plainfield. As a Hospice Care Provider, you will provide personal care services to hospice patients, assisting with daily living activities and promoting their comfort and well-being.
Key Responsibilities:
- Provide personal care services to hospice patients, including bathing, oral hygiene, hair care, and skin care.
- Assist patients with incidental and instrumental activities of daily living, such as dressing, feeding, and toileting.
- Recognize and report changes in patient condition and safety to the assigned RN.
- Document observations, activities, and services provided in a timely and accurate manner.
Requirements:
- Eighteen (18) years of age or older.
- Graduate of a training and competency program or competency program as required by State regulations.
- Ability to perform and prioritize multiple tasks or functions.
- Meets applicable health requirements to provide patient care.
- Able to effectively communicate with clinical and non-clinical individuals.
