POSITION:


As a key member of our healthcare team, the Physical Therapist is responsible for delivering comprehensive patient care, which encompasses assessment, treatment, and the development of personalized rehabilitation plans. This role emphasizes goal-oriented strategies, family education, and meticulous documentation in alignment with the standards set forth by our organization and the needs of the community.

In this capacity, the Physical Therapist acts as an advocate for patients and their rehabilitation journey, ensuring effective communication with both internal and external stakeholders. Engagement in quality assurance initiatives and customer service enhancement is also a vital component of this role.

REPORTING STRUCTURE:
This position reports directly to the Facility Rehabilitation Director.

Responsibilities of the Physical Therapist

The Physical Therapist will:
  • Deliver and oversee high-quality patient care, including assessment and therapeutic interventions.
  • Develop individualized treatment plans and set achievable goals.
  • Educate families and caregivers about rehabilitation processes and expectations.
  • Document patient progress and treatment outcomes in accordance with organizational protocols.
  • Act as a rehabilitation advocate for patients, ensuring their needs are met.
  • Participate in quality assurance and customer service initiatives.
  • Supervise the activities of assistants, rehabilitation aides, and students, adhering to state and federal regulations.

Qualifications

Education:
Must be a graduate of an accredited Physical Therapy program.
Must maintain good standing with all relevant regulatory bodies and licensing organizations.
Possess a solid understanding of Medicare and other insurance providers.

Licensure:
Must hold a current, active license as a Physical Therapist, with the ability to practice without restrictions.
CPR Certification is required for license renewal.

PHYSICAL REQUIREMENTS:
(With or Without the Aid of Mechanical Devices)

Must be capable of moving intermittently throughout the workday.

Must possess adequate vision and hearing, or utilize prosthetics to ensure these senses function effectively to meet the demands of the position.

Must fulfill the general health requirements established by the facility, including an annual TB screening and physical examination.

Must be able to lift and carry over 100 pounds with assistance, based on patient needs.
Must be able to sit and walk for extended periods.
Must be able to travel to various locations as required.
Must be able to bend, squat, push, stoop, and pull as necessary, particularly in providing patient care.
Must be able to manage the mental and emotional demands of the role.

WORK ENVIRONMENT:
Work is primarily indoors, with normal temperature conditions, some noise, and occasional exposure to fumes, odors, and chemicals, as well as potential exposure to bloodborne pathogens.


This position involves exposure to the stresses associated with a 24-hour skilled care environment, catering primarily to individuals with disabilities and cognitive impairments in an aging population.

Examples of these stresses may include:

emergency health responses, weekend and holiday shifts, unpredictable resident behavior, family dynamics related to care, end-of-life issues, oversight by regulatory agencies, and varying levels of medical staff involvement.


QUALIFICATIONS:

Education:
Graduate of an accredited Physical Therapy program.
Must maintain good standing with all relevant regulatory bodies and licensing organizations.
Knowledge of Medicare and other insurance providers is essential.
Comprehensive understanding of patient rights.

Licensure:
Must hold a current, active license as a Physical Therapist, with the ability to practice without restrictions.
CPR Certification is required for license renewal.

Experience:
Prior experience in long-term care settings is preferred.
Able to manage multiple tasks concurrently.

Language Skills:


Must be proficient in reading, analyzing, and interpreting scientific and technical information, and be able to communicate effectively in English.


Mathematical Skills:


Must be capable of performing basic mathematical operations, including addition, subtraction, multiplication, and division, using whole numbers, fractions, and decimals.

Ability to apply these skills in weight and volume measurements.

Communication Skills:
Exceptional communication and customer service skills are required, along with a strong sense of empathy.
Ability to effectively engage with patients, families, staff, and external resources.
Professionalism in all interactions is essential.
