Essential Responsibilities:- Assessment and Planning: Utilize the nursing process to conduct ongoing assessments of patient care needs, develop written plans of care, and assist patients and families in establishing health care goals.
- Coordination and Communication: Collaborate with healthcare team members to prioritize patient care, provide nursing interventions, and offer telephone consultation to patients and families.
- Aging Population Care: Demonstrate understanding of the physiological and psychological effects of normal aging, providing care that meets the unique needs of older adults.
- Patient Education and Support: Identify patients' educational needs, develop educational components for care plans, and provide teaching and counseling to promote patient self-care and adherence to medical regimens.
- Health Promotion and Prevention: Assess patients' lifestyle risk factors, integrate health promotion and preventive aspects of care, and work with patients to develop plans to improve their overall health status.
- Quality and Safety: Adhere to KFHPW documentation standards, provide a safe environment for patients, and participate in quality assurance activities to ensure high-quality care.
- Education: Associate's degree in Nursing from an accredited program
- License and Certification: Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire
- Communication and Interpersonal Skills: Possess excellent communication, problem-solving, leadership, and decision-making skills
