Medical Center Transition Coordinator

Metrowest Medical Center is seeking a dedicated and detail-oriented Medical Center Transition Coordinator to join our team.
This key role involves coordinating patient care activities, conducting psycho-social assessments, and developing individualized care plans. You will work closely with physicians, nurses, and other healthcare professionals to ensure seamless transitions and optimal health outcomes.
The estimated salary for this position is $**80,000 - 100,000 per year**. Our comprehensive benefits package includes health insurance, retirement savings plan, and paid time off.
To excel in this role, you will require a Master's of Social Work degree and at least 2 years of experience in a healthcare setting. A Certified Case Manager (CCM) certification is highly valued.
Your primary responsibilities will include assessing patients for transition needs, developing and implementing care plans, and communicating with patients, families, and caregivers to ensure smooth transitions and optimal health outcomes.
