Metrowest Medical Center is seeking a skilled and compassionate Acute Care Case Manager to join our team.

We are committed to providing high-quality, comprehensive care to our patients. As an Acute Care Case Manager, you will play a vital role in facilitating patient care along a continuum through effective resource coordination.

The estimated salary for this position is $**60,000 - 80,000 per year**, depending on experience and qualifications. We offer a competitive benefits package, including health insurance, retirement savings plan, and paid time off.

The ideal candidate will have a Master's of Social Work degree and at least 2 years of acute hospital experience. A Certified Case Manager (CCM) certification is also preferred.

In this role, you will be responsible for assessing patients for transition needs, identifying those at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.

You will work closely with physicians, office staff, and ancillary departments to ensure patient education is completed to support post-acute needs. Your documentation skills should be concise and complete, with a focus on maintaining accurate patient demographic and insurance information.
