Job Summary

We are seeking a skilled Care Navigator to join our team at Sunflower Health Plan. As a Care Navigator, you will play a critical role in developing and implementing personalized care plans for our members, ensuring they receive high-quality, cost-effective healthcare outcomes.

Key Responsibilities
  • Evaluate member needs, identify barriers to care, and recommend solutions to achieve optimal outcomes
  • Develop and coordinate care plans, working with healthcare providers and community resources to ensure timely access to services
  • Provide psychosocial support and education to members and their families, empowering them to make informed healthcare decisions
  • Collaborate with care teams to ensure seamless coordination of care and services
  • Monitor progress, identify areas for improvement, and revise care plans as needed
  • Document all member information and care management activities to ensure compliance with regulatory requirements
Requirements
  • Bachelor's degree in a related field and 2-4 years of experience in care management or a related field
  • Current state clinical license preferred
