Job Summary

We are seeking a highly motivated and compassionate Graduate Practical Nurse to join our team at Concordia Visiting Nurses. As a Graduate Practical Nurse, you will work under the supervision of an experienced Pennsylvania registered nurse to provide high-quality patient care in a home health setting.

Responsibilities
  • Provide skilled nursing care to patients in their homes, following established protocols and procedures.
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
  • Document patient progress and outcomes, maintaining accurate and detailed records.
Qualifications
  • Graduate of an approved nursing education program in Pennsylvania or any other state of the United States.
  • Temporary Practice Permit issued by the Board of Nursing.
  • Reliable, independent means of transportation to make home visits.
