Job Title: Hospice After Hours LPN

We are seeking a compassionate and skilled LPN to join our team in providing exceptional care to patients in Springfield and surrounding areas.

About the Role:
  • Collaborate with our team to deliver patient care plans and provide management support.
  • Work Monday through Thursday, 4 PM until midnight, with a focus on delivering high-quality care.

Requirements:
  • LPN license in the state you work
  • Graduate from an approved school of practical nursing
  • Two years of LPN experience, hospice preferred
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • Demonstrated experience with tablets, mobile phones, and EMR software
