About the Role:
As an After Hours Hospice Registered Nurse, you'll be responsible for evaluating patients' physical, psychosocial, and environmental needs, ensuring that each visit aligns with the patient's care plan. You'll conduct initial evaluations and continuously assess nursing needs, adapting care plans as necessary to meet changing patient conditions.
Key Responsibilities:

- Provide counseling, instruction, and guidance on patient safety
- Offer support and guidance to patients and their families
Requirements:
- Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing
- At least 1 year of experience as a nurse in a clinical care setting; home health experience is preferred but not mandatory
- Current State license as a Registered Nurse
- Ability to travel within a 50% radius of your coverage area
- Strong verbal and written communication abilities
