Job Overview

We are seeking a skilled Licensed Practical Nurse II to join our team at Evergreen Life Services. As a key member of our healthcare staff, you will be responsible for providing high-quality care to individuals with geriatric and intellectual disabilities.

About the Job

This role offers a competitive salary of $52,000 per year, based on experience, plus opportunities for professional growth and development in a dynamic and supportive environment.

Responsibilities:
  • Care Provision: Provide individualized care to individuals with complex needs, including those with seizures, Autism, and other related diagnoses.
  • Medical Documentation: Maintain accurate and up-to-date medical records, ensuring compliance with state and federal regulations.
  • Communication: Effectively communicate with physicians, families, and other stakeholders to ensure seamless care coordination.
  • Medication Administration: Administer medications safely and according to best practice guidelines.
  • Treatment Planning: Collaborate with interdisciplinary teams to develop and implement individualized treatment plans.
Requirements:
  • Education: Graduate of an approved Licensed Practical Nursing program.
  • Experience: Minimum of one year of clinically relevant experience in geriatric or intellectual disability care.
  • Skills: Strong knowledge of medical nursing procedures, first aid, and basic principles of nursing; ability to work independently and as part of a team.
