Descripción del Trabajo

El Coordinador de Atención de Enfermería (RN Case Manager) reporta directamente a un Supervisor Clínico y es responsable de la coordinación de la atención entre el personal clínico, los pacientes y sus familias/cuidadoras, así como médicos u otros proveedores de atención. Es responsable de las admisiones, evaluaciones de recertificación, transferencias/reanudaciones de atención y evaluaciones de alta, incluyendo la documentación de apoyo.

Tareas y Responsabilidades

  • Supervisa directamente a los CNAs, LPNs y RNs en el campo, lo que implica capacitación y reeducación del personal en habilidades/procedimientos clínicos cuando sea necesario.
  • Desarrolla un plan de atención individualizado para cada cliente.
  • Proporciona apoyo, orientación y educación a pacientes y sus familias/cuidadoras para ayudar a los pacientes en su progreso hacia sus objetivos.
  • Completa toda la documentación clínica de manera oportuna y precisa, asegurando que toda la documentación cumpla con los requisitos de las políticas de la agencia y las regulaciones estatales.
  • Cumple y participa en la mejora continua de la calidad reportando incidentes, lesiones e infecciones.

Calificaciones

  • Licencia estatal actual como Enfermera Registrada, se prefiere BSN.
  • Mínimo de un año de experiencia en enfermería requerida (se prefieren dos años).
  • Demostrar capacidades de liderazgo con fuertes habilidades de supervisión e interpersonales.
  • Orientado a los detalles, flexible, con buenas habilidades organizativas y de gestión del tiempo.
  • Tarjeta de CPR actual y certificado de salud actual según corresponda.
  • Licencia de conducir estatal actual y seguro de automóvil.

Información Adicional

El rango salarial para esta posición es de $70,000 a $75,000 anuales. También eres elegible para una asignación mensual de automóvil y teléfono celular que totaliza $4,200.00 anuales.
