RN - Emergency Department Case Manager

We are seeking a highly skilled RN to join our Geisinger team as an Emergency Department Case Manager. This role is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to optimize patient or member health status.
Job OverviewThis position plays a critical role in managing utilization and practice metrics to refine the delivery of care models, resulting in clinical, quality, and fiscal outcomes. The successful candidate will be responsible for integrating evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics into treatment plans that promote quality and efficiency in healthcare delivery.
Responsibilities- Develop systems of care that monitor progress and promote early intervention in acute care situations.
- Assist with the design, implementation, and evaluation of the advanced patient-centered care model.
- Assess healthcare, educational, and psychosocial needs of patients or members.
- Design individualized plans of care and foster team approaches by collaborating with patients, family, primary care providers, and other healthcare team members to ensure coordinated services.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics to monitor quality and efficiency results for assigned populations.
- Apply benefits and utilization management effectively, serving as a resource to patients and the healthcare team.
- Maintain required documentation for all case management activities.
To be successful in this role, you will need:
- A minimum of 3 years of RN work experience.
- Graduation from a Specialty Training Program-Nursing.
- Licensed Registered Nurse (Pennsylvania) certification.
