Key Responsibilities- Develop and implement individualized patient care plans, incorporating evidence-based practices and patient-centered care.
- Collaborate with interdisciplinary teams to coordinate patient care, including patient transfer, discharge, and referral needs.
- Provide education and support to patients and families, promoting health literacy and self-management skills.
- Monitor and record patient vital signs, medication administration, and clinical treatments, ensuring accurate and timely documentation.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Current state RN license and bachelor's degree from an accredited nursing school.
- Basic Life Support certification within 30 days of hire.
- ACLS and PALS certification within six months of hire.
- One year of experience preferred.
- ASLS certification preferred.
