About the Role

VNS Health is seeking a skilled Social Work Specialist to join our team. As a Social Work Specialist, you will play a critical role in providing care management services to our members, ensuring they receive the highest quality care and support.

Key Responsibilities
  • Care Management: Assess, plan, and provide intensive and continuous care management across acute, home, and long-term care settings.
  • Comprehensive Care Planning: Develop and negotiate care plans with members, families, and physicians to address their unique needs and goals.
  • Collaboration and Communication: Collaborate with interdisciplinary teams, healthcare providers, family members, and third-party payors to ensure optimum delivery and coordination of services.
  • Monitoring and Evaluation: Monitor care management activities, services, and members' responses to interventions to determine the effectiveness of the plan of care and the utilization of services.
  • Program Development: Identify trends and needs of groups in the community and plan interventions based on these identified needs.
Requirements
  • Licenses and Certifications: License and current registration to practice as a Licensed Social Worker in New York State preferred.
  • Education: Master's Degree in Social Work required.
  • Work Experience: Minimum of three years of Social Work experience required, with a minimum of two years in a case management and/or community-based environment preferred.
