Work Schedule:
Four 10-hour shifts during weekdays with participation in weekend on-call rotation.

- Take primary responsibility for assessing patient eligibility for hospice care in accordance with state and federal regulations.
- Gather comprehensive data regarding the physical, psychological, social, and spiritual needs of patients and their families, initiating interventions as necessary.
- Communicate effectively with attending physicians, interdisciplinary team members, and external agencies to facilitate admissions and recertifications for care.
- Utilize available resources efficiently to meet patient needs.
- Conduct regular visits to reassess patient eligibility, needs, and update care plans accordingly.
- Graduation from an accredited nursing program.
- Current Nurse Practitioner licensure in the state of practice.
- A minimum of two years of diverse nursing experience.
- Current CPR certification.
- Proven clinical judgment and nursing skills.
- Ability to perform home visits and deliver care in a home setting.
- Capacity to manage family emotional stress and respect individual lifestyles.
- Strong interpersonal, verbal, and written communication skills.
- Valid driver's license and proof of insurance are required.
