Senior Care Clinician

The Comprehensive Inpatient Rehabilitation Center at Curana Health seeks a skilled Senior Care Clinician to provide high-quality direct patient care services. As a key member of our team, you will administer Provider-level care, conduct clinical interventions, and evaluate patients for changes in condition.
Key Responsibilities
- Conduct clinical visits, re-evaluate care, and provide ongoing evaluation and treatment of chronic illnesses.
- Review medications, orders, and diagnostic tests, and respond to clinical abnormalities.
- Coordinate care with collaborating physicians and other disciplines, such as Therapists and Specialists.
- Maintain communication with families and attend family conferences when possible.
Requirements
- Valid nurse practitioner or physician assistant license in the state of operations and current board certification.
- Masters Degree in Nursing or Physician Assistant Studies.
- Strong leadership, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills.
