Job Summary

Key Responsibilities
  • Provide a range of psychotherapeutic and rehabilitative interventions, including psychosocial assessments, individual and group counseling, crisis intervention, and limited case management duties as needed.
  • Ensure clients receive a full spectrum of services in a seamless manner and receive the optimal level of care needed to maximize functioning and minimize the need for higher levels of care.
Responsibilities
  • Provide service coordination and case management for an assigned group of clients, including coordinating and monitoring activities of the treatment team.
  • Collaborate with the Program Manager in rotating client visits/assignments with the treatment team.
  • Develop, evaluate, and implement treatment goals and plans in conjunction with the treatment team.
  • Provide individual supportive coaching in all areas of clients' lives.
  • Perform psychosocial and risk assessments and maintain a daily log of client contacts.
  • Provide support to all clients, including on-call crisis intervention covering nighttime and weekend hours.
  • Conduct and oversee psycho-educational groups as directed by the Program Manager.
  • Perform shift management duties in coordination with other staff as directed by the Program Manager.
  • Function as an assistant to the shift manager when assigned and follow the administrative chain of command.
  • Comply with all charting and documentation requirements per contract, funding agency, and Mental Health Systems policy.
  • Prepare critical care issues for discussion during supervision and daily organizational staff meetings.
  • Collect necessary client demographic data and enter it on the Mental Health Systems Treatment Team Services Data Collection Form.
  • Provide information about the program to community agencies connected to the mental health and substance abuse services system.
  • Comply with all Mental Health Systems and program rules, policies, and procedures, including HIPAA standards.
Qualifications
  • Master's degree in a behavioral science and at least two years of experience providing clinical services to individuals with mental illness.
  • A valid CA Board of Behavioral Science license (LCSW, MFT, LPCC, or equivalent) preferred.
  • Bilingual in both English and Spanish languages preferred.
  • Valid California driver's license and current auto insurance per contract requirements.
  • Proficient user of Microsoft Office Suite and EHR software.
  • Ability to problem-solve and react appropriately to emergency situations.
  • Ability to work independently and effectively collaborate with a team.
