Job Summary

We are seeking a highly skilled Clinical Staff Pharmacist to join our team at KPH Healthcare Services, Inc. in Oklahoma City, OK. As a Clinical Staff Pharmacist, you will play a critical role in providing quality products and services to infusion therapy patients.

Responsibilities
  • Ensure compliance with federal, state, and local laws and regulations, policies, and procedures, as well as accrediting body standards.
  • Interpret and evaluate physician medication orders to ensure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness.
  • Maintain all legally required records accurately.
  • Directly supervise pharmacy support personnel and/or interns in the mixing, packaging, and labeling of pharmaceuticals.
  • Proficiently use CPR+ software.
  • Assist in training and orienting new employees.
  • Provide technical and clinical information to patients, nurses, and physicians regarding drug contents, utilization, therapeutic actions, possible adverse reactions, or interactions.
  • Initiate, review, and update all required documentation per Policy and Procedure.
  • Follow established procedures for DME and supply procurement and maintain inventory levels and preferred product selection.
  • Accurately compound IV admixtures, utilizing appropriate technique, performing checks and double checks.
  • Adhere to stability and compatibility guidelines and quality control procedures.
  • Maintain security of all legend items, including controlled substances.
  • Perform accurate computer order-entry, labeling, and dispensing.
  • Participate in clinical activities with nurses and physicians, including start of care prospective review of medical and drug histories, pharmacokinetic dosing, dosage monitoring for compromised patients, reviewing laboratory test results, attending patient care conferences, developing and updating patient-specific plans of care, and patient discharge consultations.
  • Participate in the referral acceptance/denial process and product, device, and supply selection.
  • Participate in on-call rotations.
  • Participate in Performance Improvement activities, educational programs, and departmental/branch meetings.
  • Drive the Primary Case Management Model (PCMM) in daily interactions through delegation of non-cognitive activities, such as lab packing, to support personnel, ordering clinically necessary and cost-effective labs, and facilitating positive patient outcomes with patients, nursing agencies, and other healthcare providers.
  • Perform other duties as assigned.
  • Serve as Process Owner and Compliance Officer as assigned.
  • Complete all mandatory and regulatory training programs.
Qualifications
  • Bachelor's Degree (BS) in Pharmacy or a Pharm.D. from an accredited college or university.
  • Prefer one year of experience in an IV alternate site environment or equivalent work experience.
  • Current license for state of practice.
  • Current CPR certification if involved with direct patient contact.
  • Valid driver's license and appropriate automobile insurance coverage per AristaCare's policy.

Required Training
  • HIPAA Privacy Course.
  • HIPAA Security Course.
