ABOUT THE PROGRAM:
Assertive Community Treatment (ACT) is a proven approach designed to enhance the quality of life for individuals with serious mental health challenges. Our multidisciplinary team collaborates to provide comprehensive services, including life skills training, vocational support, substance use counseling, medication management, and therapeutic interventions. If you thrive in a team-oriented environment and are passionate about making a difference in the lives of clients, this role is for you.

KEY RESPONSIBILITIES:
  • Conduct thorough clinical and risk assessments.
  • Work collaboratively with clients to develop personalized treatment plans and monitor their progress.
  • Facilitate individual and group therapy sessions.
  • Utilize motivational techniques to help clients achieve their recovery goals.
  • Provide psycho-educational services to support recovery from mental health and substance use disorders.
  • Assist clients in connecting with community resources and support systems.
  • Develop safety plans and respond to crisis situations effectively.
  • Teach essential life skills, including housing maintenance, transportation use, and nutritional planning.
  • Engage in family education and support initiatives.
  • Maintain accurate documentation in accordance with agency standards.
  • Participate in team meetings, supervision, and training sessions.
  • Fulfill additional responsibilities as required.
QUALIFICATIONS:

Education: Master's degree in a behavioral health or related field.

Experience: Minimum of 1 year working with adults facing serious mental health issues or co-occurring disorders; outreach experience is preferred.

Licensure: LMSW, LPCA, or LMFTA required if eligible; LCSW or LPC preferred.
