About the Role

As a Case Manager at Virginia Mason Franciscan Health Rehabilitation Hospital, you will play a vital role in the interdisciplinary team caring for patients who require continued care to recover completely. Key Responsibilities
  • Complete departmental orientation, initial, and annual competencies to ensure you have the necessary skills and knowledge to excel in your role.
  • Collaborate with other departmental staff to deliver patient care, identify barriers to care, and develop solutions to ensure the best possible outcomes.
  • Document patient information per workflow timeline and content requirements, including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedule family conferences and communicate with caregivers to keep patients and their families informed of progress and provide information related to goal achievement, course of rehabilitation stay, and plans for discharge.
  • Coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans to achieve identified patient goals and outcomes.
  • Review patient assigned CMG and identify potential missed comorbid conditions that are actively being treated during the patient's stay. Communicate findings to the HIM team.
  • Perform other duties as assigned.
Requirements
  • Masters or Bachelors in Social Work or as required by state.
  • Certification in Case Management or Rehabilitation Nursing preferred.
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred.
  • Effective oral and written communication skills in English, additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.
Compensation

The salary range for this position is $35.00 USD to $45.00 USD per hour. We offer a competitive compensation package that includes benefits, professional development opportunities, and a supportive work environment.
