The Transplant Surgery Advanced Practice Provider (APP) will collaborate with a distinguished team of Abdominal Transplant Surgeons and proficient Advanced Practice Providers dedicated to providing outstanding care for patients with liver, kidney, and pancreas diseases requiring transplantation. The APP will deliver thorough clinical management to both transplant recipients and donors throughout the pre-, peri-, and post-transplant care continuum.

Position Summary

The APP will primarily operate in an outpatient environment, managing both acute and chronic conditions, with a consistent clinic schedule for pre-transplant assessments, patients on the waiting list, and postoperative follow-ups. The patient demographic will encompass complex surgical conditions affecting the liver, pancreas, and kidney. Additional duties will include educating patients about their conditions and the transplantation process, as well as providing pre- and post-operative care and discharge instructions.

The Outpatient APP will predominantly be assigned to the outpatient clinic while also participating in weekend rotations for inpatient acute care rounds and occasional on-call duties during weeknights and weekends. The ideal candidate will possess the experience and expertise necessary to independently initiate and complete assessments and evaluations for surgical or transplant patients.

Key Responsibilities

  • Primarily operates in an outpatient capacity but is capable of providing care across all phases, including inpatient settings (both pediatric and adult).
  • Engages in surgical evaluations and oversees peri-operative and post-operative care for living donors.
  • Conducts initial and follow-up medical histories and physical examinations.
  • Documents findings from examinations, assessments, and care plans.
  • Completes evaluations for transplant eligibility and ongoing assessments while patients are on the waiting list.
  • Participates in care management discussions for pre-, perioperative, and postoperative planning.
  • Maintains vigilance in patient monitoring and responds effectively to potential complications.
  • Describes assessment and management strategies for acute trauma cases.
  • Responds appropriately to emergency situations.
  • Assists with new patient consultations and admissions.
  • Conducts daily assessments of patients in intensive care, progressive care, and general floor settings.
  • Collaborates with a multidisciplinary team to determine treatment plans for complex and high-risk transplant patients.
  • Prescribes medications and administers treatments as necessary.
  • Orders and interprets laboratory and radiology tests on a daily basis.
  • Records all patient interactions, documenting observations, interventions, and actions in a medically appropriate manner within the patient's medical record.
  • Participates in group meetings and contributes to the development of standard procedures for the department.
  • Engages in evidence-based research and publication efforts aimed at enhancing the care of transplant patients.

Qualifications

Possession of or eligibility for a DEA license is required. Candidates must be authorized to practice as a Nurse Practitioner or Physician Assistant in their respective state. Eligibility for prescriptive authority through the Board of Nursing or Medical Board in North Carolina is necessary. A BLS certification for Healthcare Providers from AHA is required, with an expiration date extending beyond 90 days from the start of practice. Candidates should demonstrate strong critical thinking skills, effective interpersonal communication, and the ability to prioritize and make proactive decisions.

Nurse Practitioner: A Master's Degree or Doctor of Nursing Practice is required. A valid Registered Nurse license is mandatory. Certification in the relevant practice area from a nationally recognized certifying body is essential. Acute Care board certification is required based on the population and acuity served.

Physician Assistant: Graduation from an accredited Physician Assistant program is required, with a master's degree preferred. Certification from the National Commission on Certification of Physician Assistants is preferred.
