Salary: $55,000 - $70,000 per year,

Job Summary:
We are seeking a skilled Community Care Coordinator to join our team. The ideal candidate will have experience working with homeless individuals and be able to provide supportive communication on incarceration, substance abuse, mental health, and risk behaviors.

Key Responsibilities:
  • Coordinate with community agencies and providers to ensure seamless care transitions.
  • Develop and maintain relationships with key stakeholders to advance our mission.
  • Conduct outreach and engagement activities to promote our services and build trust within the community.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Provide education and resources to clients on healthy coping mechanisms and stress management techniques.

Qualifications:
  • Minimum high school diploma or equivalency.
  • Associates Degree in Social Services preferred.
  • Previous experience working with homeless persons and mentally ill populations.
  • Strong ability to communicate effectively with diverse populations.
  • Bilingual (English/Spanish) required.
  • Experience handling confidential documents and detail-oriented.
  • Ability to use email, database program(s) of agency, and working knowledge of word processing (Microsoft Word) and spreadsheet programs.
