Case Manager

The Case Manager in the Community Support for Families (CSF) Program provides community-based, in-home services to support families in need and connect them to resources that help them achieve their goals. This role involves facilitating ongoing communication and advocacy between families, DCF, schools, landlords, medical providers, and other involved supports or service providers.
Key Responsibilities:
- Develops case management plans and provides direct case management services to assigned clients and families.
- Provides front-line crisis support to individuals and families in the program.
- Coordinates treatment phases with internal and external service providers and family supports.
- Maintains records of counseling, coordination of care, child and family team meetings, and case management activities.
- Supports daily program-specific operational functions and may transport clients to appointments or linkages as needed.
Requirements:
- BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, or a related field.
- Previous experience working with clients with mental health and/or co-occurring substance use disorders.
- Effective time management, strong verbal and written communication skills, and a valid driver's license.
