About the Role:

We are seeking a dedicated and compassionate Direct Support Provider to join our team at Prairie Community Services. As a Direct Support Provider, you will play a vital role in supporting individuals with disabilities in a residential setting.

Key Responsibilities:

  • Provide training and assistance to individuals with daily personal cares, medication administration, community activities, money management, meal preparation, laundry assistance, and leisure activities.
  • Monitor and assist individuals with personal cares, including bathing, dressing, and grooming.
  • Encourage independence and self-advocacy among individuals served.
  • Communicate effectively with individuals, families, and healthcare professionals to ensure seamless care.
  • Chart daily program information and maintain accurate records.

Requirements:

  • Must be 17 years of age or as dictated by licensure.
  • Must pass DHS background clearance.
  • Must have valid driver's license.
  • Must be able to read, write, comprehend, and communicate in English.
