Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Patient Care Coordinator. This role will provide exceptional care to our patients and their families, ensuring a seamless and personalized experience throughout their treatment journey.

Key Responsibilities
  • Document nursing assessments and patient teaching in the patient chart
  • Communicate all pertinent information and any interventions to the physician
  • Provide professional nursing care for patients following established standards and practices
  • Act as a resource person to the medical assistants
  • Triage patient phone calls as needed and document relevant information in the patient's chart
  • Assess patient/family needs and provide teaching and other instructional material
  • Manage patient medication refill requests from the pharmacy
  • Participate in professional development activities and maintain professional affiliations
  • Work closely with the healthcare team to ensure patient care needs are met
Requirements
  • Graduate from an accredited nursing program, BSN preferred
  • Current MN licensure and registration required
  • Current or pending OCN certification preferred
  • Current CPR certification required
  • 2-3 years of nursing experience with IV experience preferred
