Job Title: Nurse Navigator/RN

Join Northern Light Health as a Nurse Navigator/RN and play a vital role in ensuring timely access and seamless coordination of services throughout the continuum of care.

Job Summary:

The Nurse Navigator serves as a consistent care coordinator for the diagnostic and surgical end of Urologic care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, physicians, and other healthcare professionals.

Responsibilities:
  • Clearly and effectively communicate with patients, families, physicians, and fellow employees
  • Respond to patient-generated concerns and complaints regarding nursing care
  • Support management team efforts and use a constructive, consistent approach in the oversight of staff
  • Serve as a resource to staff delivering services to patients and provide follow-up on patient, family, and physician complaints
  • Develop and maintain positive working relationships and working environment
  • Efficiently coordinate scheduling of staff and provide input on staff performance
  • Assist Department Head with counseling and disciplining of staff and monitor corrective action/performance improvement
  • Adhere to job description and established performance standards
  • Serve as a resource to all members of the healthcare team and perform all tasks of Staff Nurse
  • Develop and implement triage standards, including proper telephone etiquette
  • Collaborate with other disciplines based on patient assessment and identified needs
  • Complete assigned or requested tasks in a timely, efficient manner
  • Coordinate patient activities within the department and/or among departments
  • Actively work to creatively problem-solve to meet service and program needs
  • Provide day-to-day supervision of clinical operations and personnel performance
  • Assist in coordination of educational opportunities for staff development
  • Develop and maintain through effective collaboration a competency-based orientation program
  • Coordinate the development, implementation, and evaluation of patient/family education programs
  • Perform all other duties as assigned
Requirements:
  • Required: Registered Nurse
  • Education: Associate's Degree
  • Working Conditions: Potential exposure to noise levels being uncomfortable
